    "description": "<p>ABSTRACT Background:Anemia was defined based on age-specific, altitude-adjusted hemoglobin thresholds. It is a common hematological complication among children receiving highly active antiretroviral therapy (HAART) and has been identified as a predictor of early mortality in HIV-infected pediatric cohorts on HAART. Objective: The aim of this study was to determine themagnitude of anemia and its associated factors among HAART experienced HIV positive children. Methods:A facility-based prospective cross-sectional study was conducted in Illubabor Zone, Southwest Ethiopia fromDecember, 2025, to February, 2026among 198 HAARTexperienced children. Data were collected through interviews and medical record review. Anemia was determined from venous blood using an automated hematology analyzer. Data were entered into EpiData version 4.6 and analyzed using SPSS 26. Binary and multivariable logistic regression were used, with p&lt; 0.05 considered significant. Result: The magnitude of anemia among participant were 30.2% (95% CI: 23.9&ndash; 37.4, n=60).Normocytic normochromic anemia was the predominant morphological pattern in the current study, accounting for 63.7% of cases, followed by microcytic hypochromic anemia (26.7%) and macrocytic normochromic anemia (10%). Children aged &le;7 years had 4.2-fold higher adjusted odds of anemia compared with those aged 14&ndash; 19 years (AOR 4.2, 95% CI: 1.9&ndash;9.1), while those aged 7&ndash;14 years had elevated but non-significant odds (AOR 1.8, 95% CI: 0.8&ndash;4.1).Taking additional medications or supplements increased anemia risk (AOR 7.8, 95% CI: 1.46&ndash;42.5).Low CD4 count was associated with increased odds of anemia(AOR 11.3, 95% CI: 1.22&ndash;103.2), although the estimate was imprecise xv Conclusion and Recommendation:This study shows anemia remains common among HAART-experienced children despite high viral suppression (90.4%), indicating that viral control alone is insufficient. Routine hematological monitoring and targeted anemia management should be part of their clinical care, and further studies are needed on its causes and complications.
